Here is the link for a Family Goal Form that you can use to document family support at home. It's not a contract or a binding document, but QUEST prides itself on this 3-way relationship with student-teacher-families. This may be a simple way for you to electronically gather that information and for parents to focus in on the supports they provide their child for educational success. **You will need to "make a copy" of the form (and rename it with your name) before sending it to parents in order to gather your OWN data. Otherwise, all the info we gather will be produced as a school-wide .csv file and you'll have to sift through many rows of data to check out parent responses.
